HIPAA Notice of Privacy Practices for Holly Family Dental

Introduction:

Holly Family Dental is committed to protecting the privacy of our patients’ health information in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). This HIPAA Notice of Privacy Practices outlines how we may use and disclose our patients’ protected health information (PHI) for treatment, payment, and healthcare operations. It also explains our patients’ rights with respect to their PHI, and how they may exercise those rights.

Uses and Disclosures of Protected Health Information (PHI):

We may use and disclose a patient’s PHI for the following purposes:

Treatment: We may use a patient's PHI to provide them with dental care and related services. This may include sharing the information with other healthcare providers involved in the patient's care.
Payment: We may use a patient's PHI to obtain payment for the services we provide. This may include sharing information with insurance companies, Medicare, or Medicaid.
Healthcare Operations: We may use a patient's PHI for the administration and management of our practice, including quality improvement, auditing, and regulatory compliance.
Other Permissible Uses and Disclosures: We may use or disclose a patient's PHI in accordance with other applicable laws, such as for public health activities, abuse or neglect reporting, or as required by law or court order.

Patients' Rights:

Under HIPAA, our patients have certain rights with respect to their PHI. These rights include:

Right to Request Restrictions: Patients have the right to request restrictions on how their PHI is used or disclosed for treatment, payment, or healthcare operations. We are not required to agree to these restrictions, but if we do, we will abide by them unless emergency circumstances require otherwise.
Right to Receive Confidential Communications: Patients have the right to request that we communicate with them in a confidential manner, such as by mail or email.
Right to Inspect and Copy: Patients have the right to inspect and receive a copy of their PHI in our possession. There may be limited circumstances in which we are not required to comply with this request.
Right to Amend: Patients have the right to request that we amend their PHI if they believe it is inaccurate or incomplete.
Right to an Accounting of Disclosures: Patients have the right to receive an accounting of certain disclosures of their PHI that we have made in the six years prior to the request.

If a patient believes their privacy rights have been violated, they may file a complaint with our Privacy Officer or with the Secretary of the U.S. Department of Health and Human Services.

Amendments to this HIPAA Notice:

We reserve the right to amend this HIPAA Notice at any time. In the event of an amendment, we will provide a revised notice to our patients and post it in a prominent location in our office.
